Are you a Mandarin- or Cantonese-speaking healthcare professional looking for a rewarding Care Management opportunity? We're hiring a Care Manager to serve as the primary point of contact between members, their families, providers, and the health plan.

$20,000 SIGN-ON BONUS

Make a direct impact on members and their families

What You'll Do:

• Build relationships with members through monthly communication and ongoing follow-up

• Review person-centered service plans with members and their families

• Coordinate covered and non-covered benefits and services

• Collaborate with families, providers, and vendor agencies to ensure continuity of care

• Identify member needs and make sure concerns are addressed promptly

• Maintain timely and thorough documentation in member charts

• Participate in weekly interdisciplinary team meetings

What We're Looking For:

• Fluency in Mandarin or Cantonese is REQUIRED

• Strong communication and relationship-building skills

• Knowledge of member assessments and clinical xhyhwjd review procedures

• Ability to work effectively with members from diverse backgrounds and age groups

• Organized, compassionate, and comfortable coordinating multiple aspects of member care
