The role is integrated within a hospice center setting, where you will engage with a diverse group of individuals, including patients and their families, to support their emotional and psychological needs during end-of-life care.

Key Responsibilities\n

• Conduct thorough psychosocial assessments to evaluate clients’ mental health concerns, strengths, and resources, ensuring a comprehensive understanding of their needs.

• Provide individual, family, and group therapy rooted in evidence-based treatment modalities, such as cognitive-behavioral therapy and trauma-focused therapy.

• Develop personalized treatment plans that set clear, achievable goals for clients, integrating feedback from interdisciplinary team members.

• Collaborate closely with psychiatrists, psychologists, and other healthcare professionals to devise and implement effective client-centered treatment plans.

• Offer crisis intervention, risk assessments, and safety planning as needed, ensuring clients feel supported and secure.
\n
• Facilitate group therapy sessions and psychoeducation classes, creating a safe space for clients to express themselves and learn coping strategies.

• Maintain accurate and timely client documentation to comply with both ethical and legal standards, ensuring a record of all interactions and treatment decisions.

• Engage in continuous professional development and partake in ongoing training to enhance clinical skills and knowledge.

Patient Population / Client Focus\n

In this role, you will primarily work with individuals experiencing mental health challenges related to grief, loss, and end-of-life transitions. You will focus on providing emotional and psychological support to patients within a hospice setting, ensuring that they receive compassionate care tailored to their unique situations. By addressing complex emotional issues and facilitating therapeutic engagement, you will play a key role in improving the well-being of both patients and their families during difficult times.

Qualifications\n

• A Master’s degree in Social Work from an accredited program is required.

• Active licensure as a Licensed Clinical Social Worker (LCSW) in the state of California.

• Minimum of two years of experience in mental health or outpatient settings, particularly with hospice or palliative care is preferred.

• Strong knowledge of evidence-based therapeutic approaches and an ability to implement them effectively with diverse clients.

• Excellent interpersonal skills and the ability to communicate compassionately and effectively with clients, families, and colleagues.

Compensation and Benefits\n
