Outreach Clinical Social Worker I/II - Housing Stability

UCSF Health is seeking a Clinical Social Worker I/II to perform outreach in San Francisco's Tenderloin, South of Market, and Mission neighborhoods, engaging formerly homeless adults and linking them to mental health, substance-use, and medical services. You will conduct assessments, provide case management, document interventions in Epic, and work with a multidisciplinary team to reduce eviction risk and support housing stability. #J-18808-Ljbffr UCSF Health
