Hospice Team NurseLocation: California Bonus: Eligible for Sign-On BonusRole Overview As a Hospice Team Nurse, you'll provide personalized, in-home hospice care, guiding patients and families through their journey with empathy and clinical excellence. You'll collaborate with physicians, interdisciplinary teams, and community health professionals to ensure seamless, compassionate care.Key Responsibilities:Assess and evaluate patient statusDevelop and revise individualized care plansProvide palliative nursing care and educationAct as Case Manager when assignedMaintain accurate clinical documentationCommunicate with physicians and community partnersQualificationsHomecare Homebase/Point Care EMR experience is an added bonus.This role is ideal for someone who thrives in a patient-centered environment and values meaningful connections.
