Lead Clinical Coordinator

Behavioral Health Services – ACT Team

Hourly Range: $30.19 - $45.29

Job Summary : The Lead Clinical Coordinatorwill provide clinical oversight, ensure program growth, and follow Community Support Program (CSP) statutory requirements under DHS 63 for admission, clinical supervision, crisis management, and ongoing case management. This position will conduct intake assessment and planning, crisis management/planning, treatment planning, coordination of care, clinical supervision of staff, and provide comprehensive, direct, clinical care to individuals with serious and persistent mental illness.

Job Duties:

• Conduct individual assessments, including in-depth assessment at time of intake. Complete routine assessments thereafter at least every six months. Provide oversight of staff assessments and planning, ensuring alignment with DHS 63 standards, and establish rehabilitative goals to further recovery.
• Provide direct case management services to help consumers successfully live in the community. Provide brokerage of services outside of ACT team, regular collateral contacts of providers, and others involved in the consumers' lives.
• Provide crisis response services to individuals who are experiencing a psychiatric crisis. Interface with county operated crisis services, hospitals, and others as needed. Includes being part of a rotation of after-hour on-call services provided by the ACT team.
• Provide individual and group therapy as well as psychoeducation groups to ACT team consumers by using Motivational Interviewing, Cognitive Behavioral Therapy, Dialectical Behavioral Therapy, and other evidence based therapeutic practices. Enhance the consumers’ independent living skills.
• Complete clinically appropriate and timely documentation of all services rendered on behalf of consumers served by the ACT team in the Electronic Medical Record. Maintain a productivity level of 75% or greater.
• Support the Integrated Services Manager to lead daily team meetings, help resolve clinical problems, ensure quality of treatment provisions, and oversee staff accountability.
• Provide clinical supervision to team members who are working toward achieving supervised hours so they can obtain licensure. Provide clinical support to all team members to navigate paths of recovery and clinical interventions for ACT team clients.
• Travel of up to 75% of time within the community is required.
• Requires being part of a rotation for after-hours and weekend on-call services.
• Perform other duties as assigned to support the care and services of clients being served by the ACT team.

Minimum Requirements:

• Must be a current resident of the state of Wisconsin, or be able to establish residency within six months of the start date, and maintain residency for the duration of employment.
• Valid Wisconsin driver’s license and access to a reliable insured vehicle, both maintained throughout employment
• Master’s Degree in Human Services, Psychology, Social Work, Occupational Therapy, or in Nursing (RN) is required.
• Licensure as a LPC, LCSW, LMFT is required.
• Either 3,000 hours of supervised clinical experience in a practice where the majority of clients are adults with chronic mental illness or 1,500 hours of supervised clinical experience in a Community Support Program is required
• At least five (5) years of related experience providing services to individuals with Mental Health and Substance use disorders is required.
• At least intermediate experience with all related computer and software such as Microsoft Office Suite programs
• Available and willing to be part of a rotation for after-hours and weekend on-call services as well as travel within the community up to 75% of work time.
• Clinical knowledge of the needs of individuals with serious and persistent mental illness, co-occurring disorders, and the civil legal system is required.

Knowledge, Skills, and Abilities: Knowledge of and experience in community mental health, community support programs, and DHS 63. Knowledge and experience utilizing the Evidence Based Practice of Assertive Community Treatment and Motivational Interviewing. Knowledge of and skill in the application of ethics, theories, methods, practices, treatments and techniques of social work; knowledge of community resources in the area of health and social services and the ability to utilize them effectively; knowledge of mental health and substance use disorders and diseases; knowledge of laws governing mental health commitments; skill in interviewing patients and family members and accurately assessing their needs; ability to work as a member of a mental health or health care team; ability to work with people of all social, economic and educational levels; ability to develop effective interpersonal relationships; ability to work effectively and harmoniously with staff, patients, families, and agencies; oral and written communication skills; able to life/carry/push/pull up to 25lbs as needed.

NOTE: This original application is open to permanent residents of the United States. Appointee must establish residency in the state of Wisconsin within six months of start date and maintain such residency during incumbency. In accordance with Section 11, Rule 3 of the Civil Service Rules a non-competitive examination will be held for this position. The non-competitive examination will consist of criteria deemed appropriate by the Director of Human Resources.
