Rural Health Transformation Office (RHTO) Grant Program Manager Job Location: Howerton State Office Building, 615 Howerton Court, Jefferson City, MO. Job Posting Number: 4577 8969093 Annual Salary: $75,000.00 Semi-monthly Salary: $3,125.00 (may increase based on pay differential). Why you’ll love this position The RHTO Grant Program Manager for the Rural Health Transformation Program (RHTP) will provide day-to-day leadership and operational oversight for this critical component of Missouri’s RHTP. Reporting directly to the RHTP Director, this role serves as a core leader within the newly established Rural Healthcare Transformation Office (RHTO), responsible for driving strategy, governance, performance management, and capability building across the five-year, $500M–$1B program. Responsibilities Define and drive the full implementation roadmap across a portfolio of sub-initiatives, orchestrating execution across a diverse stakeholder landscape while proactively managing risks and dependencies. Ensure that sub-initiatives are delivered efficiently, effectively, and in alignment with program-wide objectives through disciplined oversight of timelines, budgets, and performance. Own the development and ongoing maintenance of the program-wide workplan. Monitor progress against milestones, identify variances and mobilize resources rapidly to address delays and emerging risks. Lead the development and refinement of KPIs for the program overall and for specific initiatives. Establish and maintain program-wide dashboards and underlying data infrastructure across stakeholders to ensure a single source of truth for program metrics. Oversee Grant Management Vendor to ensure effective grant administration and sound financial management across all pillars. Translate complex data into executive-ready insights and reports, including regular updates to CMS and other state and federal stakeholders. Orchestrate the program's operational cadence, leading preparation for and facilitating performance management reviews. Establish and run a robust knowledge and best practice sharing model across RHTP initiatives. Identify and resolve complex challenges by navigating ambiguity, addressing root causes, and removing roadblocks to maintain program momentum. Drive execution across stakeholders by coordinating teams, maintaining alignment, and ensuring initiatives progress effectively against timelines and objectives. Other duties as assigned. Minimum Qualifications Bachelor’s degree, 5‑7 years of relevant experience, and 2‑4 years of supervisory experience, preferably in healthcare data analysis, quality improvement (QI), performance management, or health informatics. Demonstrated ability to manage complex, multi‑workstream projects and deliver results against defined milestones. Strong analytical skills with experience translating data into actionable insights for leadership and external audiences. Preferred Qualifications Experience working within state or federal grant‑funded programs, with a strong understanding of regulatory compliance requirements. Proven track record of leading organizational change or health system transformation efforts. Experience designing and implementing performance management frameworks, specifically within a government or social services context. Proven ability to synthesize data from disparate sources to inform strategic planning and decision‑making. Proficiency in data visualization and reporting tools (e.g., Tableau, Power BI) and statistical analysis platforms (e.g., SQL, SAS). Understanding of the full procurement lifecycle, from drafting Requests for Proposals (RFPs) to contract negotiation and vendor performance monitoring. Project Management Professional (PMP) certification or equivalent methodology training. Analytical and detail‑oriented with a passion for data‑driven decision‑making. Highly organized with a strong focus on process, timelines, and operational rigor. Structured problem‑solver comfortable operating in ambiguous, fast‑moving environments. Collaborative and communicative across diverse internal and external stakeholders. Relationship‑builder who can drive accountability without direct authority.
