Work Environment As a Physical Therapist with Evergreen, you will work onsite in patients' homes throughout Old Bridge and the surrounding areas. Patient Population / Client Focus Your primary focus will encompass a diverse patient population, including individuals recovering from surgical procedures, those managing chronic pain conditions, and elderly patients requiring rehabilitation and support. By delivering personalized therapy in their homes, you will help minimize barriers to care, provide comfort, and promote independence. Qualifications * A Bachelor's degree in Physical Therapy or a related field is required. * Active state license to practice as a Physical Therapist in New Jersey. * Current CPR certification from the American Heart Association is required. * Strong interpersonal skills to build rapport with patients and collaborate effectively within a multidisciplinary team. Preferred Experience * A minimum of two years of experience in physical therapy, preferably in a home care setting. * Familiarity with documentation systems and electronic health records (EHR) to ensure accurate and timely patient information management.
