Job Details – Travel Nurse / Home Health RN Assignment Type: Travel (Home Health RN, Travel Nurse) Location: Portland, OR 97219 (serving a 60-mile radius from Salem) Weekly Estimated Pay: $2,120 – $2,230 Start Date: May 4, 2026 End Date: August 31, 2026 Duration: 17 weeks Shift: Days, 8:00 AM – 4:30 PM Hours per Week: 40 (guaranteed) On Call/Weekend Requirement: Yes Travel: Mileage reimbursed between patient visits Key Qualifications – Registered Nurse (RN) Active RN license in the state of Oregon (required) Current CPR certification (BLS preferred) Valid driver's license and reliable transportation Associate's Degree in Nursing or higher Minimum 1 year of nursing experience required (home health experience preferred) Experience with wound care and HCHB EMR is a plus Strong clinical assessment, patient education, and care coordination skills Responsibilities – Home Health Nursing / Community-Based Care Conduct Start of Care (SOC) visits, recertifications, evaluations, and Resumption of Care (ROC) visits Provide subsequent skilled nursing visits and ongoing home health nursing care in patients' homes Deliver compassionate, patient-centered care to a diverse community across Portland and surrounding areas Document all care accurately and promptly using electronic medical records (EMR/HCHB) Collaborate with interdisciplinary teams, case managers, and physicians to ensure optimal patient outcomes Participate in on-call and weekend rotations as required Why Join This Travel Nursing Opportunity?
