Job Description Summary The Nursing Assistant II/UC assists in providing direct patient care throughout nursing areas, and is responsible for the safe transport of patients via wheelchair or stretcher, and other devices. The NA II/UC performs basic care tasks as delegated by the RN, and is responsible for appropriate documentation of services. Provides the services of a Patient Observer (Sitter) for any patient who has been assessed to require constant observation in order to maximize his/her safety. The NA II/UC needs to understand the patient’s condition to ensure their care and safety. In addition to other tasks, the NA II/UC performs phlebotomy (venipuncture, finger stick and heel stick) according to established procedure of South Shore Hospital. Must successfully perform 50-100 draws to be deemed competent by a designated phlebotomist trainer. Once deemed competent, the NA II/UC shall continue to perform phlebotomy duties on a consistent basis (typically on a daily basis), and successfully complete an annual competency assessment. The NA II/UC will also function in the role of Unit Coordinator. Unit Coordinator/Secretary through communication, requisitioning and management of supplies and equipment, coordinates the activities of the nursing staff, the patient, the physician, other Hospital departments and visitors. He/she performs general receptionist and clerical duties in order to facilitate efficient functioning of the nursing unit. Job Description

ESSENTIAL FUNCTIONS

1 - Patient Care Demonstrates the skills and judgment necessary to provide direct/non-direct care to patients under the direct supervision of licensed personnel. Observes, obtains and reports patient data/status to assigned RN. Responds to peaks in acuity and emergent situations as directed by nursing staff. 2 - Professional Development Maintains current knowledge/certification. Maintains BLS certification. Functions as a preceptor to new employees. Displays the ability to accept and respond appropriately to feedback and recommendations for change. Demonstrates professional working relationships with colleagues from all disciplines to promote a positive/encouraging workplace. Accountable for being informed about changes in hospital policy and procedure. Utilizes critical thinking and problem solving in day to day operations as well as emergent situations. Communicates appropriately and effectively utilizing the chain of command. 3 - Safety/Quality Fosters a "Culture of Safety" through personal ownership and commitment to a safe environment. Verifies patient identification with 2 identifiers prior to the start of any procedure. Understands individual role/responsibilities in the event of hospital codes and emergency preparedness. Complies with the current CDC hand hygiene guidelines through proper handwashing. Adheres to universal precautions, makes appropriate use of personal protective equipment at all times and appropriately disposes of hazardous materials. Maintains awareness of MSDS sheets and how to access. Demonstrates awareness of each patient’s fall risk and the appropriate use of safety devices and identified safety interventions. Conducts patient safety rounds utilizing principles of SBAR, follows chain of command to communicate any identified patient or staff safety risks. Demonstrates proper body mechanics for all functions and use of patient lifts as needed. 4 - Technology Utilizes technological solutions to work processes and practices. Utilizes software applications required by department and unit standards. Accountable to understand how to operate in downtime. 5 - Orders/Admission/Discharges/Transfer of Patients Scans necessary orders to Pharmacy and alerts nurse to STAT orders. Reviews and follows up on all CPOE orders and order entry per SSH policy and procedure. Keeps bed track and PCS systems up to date. Books appointments through central scheduling as indicated. Ensures discharge disposition is entered accurately and in a timely manner. Fax necessary discharge paperwork and medication reconciliation to appropriate provider(s). 6 - Unit Workflow Prints, reviews and inquires about all unit reports as needed. Files reports in the patient's chart in appropriate areas. Completes accurate daily chart audits. Completes a shift to shift report (ie: NPO patients). Completes shift rounds (ensure precaution rooms match med track and door tags are correct). 7 - Supplies/Services Orders supplies from Stores/other departments as needed or requested in collaboration with Operations Supervisor. Collaborates with Operations Supervisor to maintain/change par levels and to order from outside vendors. 8 - Compliance Works within legal, regulatory and ethical practice standards relevant to the position. Complies with applicable policies and procedures. Safeguards the privacy and security of patient information. The employee complies with policies and procedures relating to SSH’s privacy and security programs. Brings potential compliance issues to a manager, supervisor, director or VP. Complies with the mandatory education requirements of the compliance, privacy and security programs. 9 - Patient and Family Centered Care Conveys respect for values, preferences, and expressed needs of the patient and family. Recognizes the patient, and family according to patient preferences, as important members of the health care team. Collaborates with the patient, and family according to patient preferences, in planning, implementing, and evaluating care. Welcomes the presence and participation of family members at all times according to patient preferences, regardless of rounds, change of shift, or other events on unit. 10 - Age & Culture Considers the individual needs of each person with whom they interact. Possesses age and cultural knowledge and awareness. Interacts with sensitivity in the delivery of care/services of diverse populations as needed. Effectively utilizes resources to provide care/services - such as, interpreter services and online sources.

JOB REQUIREMENTS

Minimum Education High School Diploma/GED preferred. Minimum Work Experience Previous Nursing Assistant or equivalent experience preferred. Knowledge of basic computer knowledge Knowledge of basic medical terminology Required Classes/Skills BLS - Basic Life Support Required additional Knowledge, and Abilities Knowledge of basic medical terminology.
