Key Responsibilities\n

• Conduct thorough intake and psychosocial assessments for newly admitted patients, evaluating their emotional and social needs.

• Develop tailored treatment plans and present recommendations during clinical care review meetings, in conjunction with the Chemical Dependency Recovery Program (CDRP) team.
\n
• Participate actively in clinical case conferences to discuss patient progress and necessary modifications to treatment approaches.
\n
• Deliver educational lectures focused on chemical dependency, enhancing knowledge and awareness among patients and their families.
\n
• Facilitate individual and group counseling sessions, focusing on chemical dependency and supporting recovery journeys.
\n
• Provide family therapy, including adolescent, significant other, and multi-family therapy, ensuring a holistic approach to patient care.
\n
• Offer comprehensive case management services, including resource coordination and referrals for additional support and community services.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker in our hospice center, you will primarily work with patients facing terminal illness and their families. Your role will be crucial in addressing not only the emotional aspects of end-of-life care but also providing support within the context of chemical dependency and mental health challenges that may arise. This population requires sensitivity, a deep understanding of grief and loss, and the ability to offer counseling that is both compassionate and effective.

Qualifications\n

• A Master's Degree in Social Work (MSW) or a closely related field is required.

• Candidates must possess an active LCSW license in the state of Oregon.

• A minimum of two years of relevant supervised experience in addiction medicine therapy within the past five years is essential.

Compensation and Benefits\n
