In this role, you will primarily be creating and managing a plan of care for individual patients. As an RN, you demonstrate great attention to detail and a caring heart. You take pride in creating personalized care plans and can observe and communicate effectively in your patient’s best interest. Service area: Port St. Lucie to Sebastian, FL. Flexible schedule and hybrid work opportunities available. Responsibilities Coordinate with other colleagues to assess, plan, implement, or evaluate patient care plans Oversee and support in‑home caregiver teams Ensure compliance with care plans and agency standards Document any changes in patient conditions and report on changes Maintain accurate records of patient care and discuss observations with the supervisor Qualifications Currently licensed Registered Nurse (RN) in the state of employment Previous experience as an RN is preferred Excellent interpersonal and client care skills Valid driver’s license First aid/CPR certification required English required; Spanish and Creole speakers a plus Reliable transportation Ability to pass a Level 2 Background Check and I-9 verification Flexible work from home options available. #J-18808-Ljbffr Kenner + Imparato PLLC
