Addiction Treatment Prg Camden, NJ 08104, USA Description General Functions: The employee in this classification shall report to the Treatment Coordinator. The employee is part of the treatment team. The employee is responsible for fulfilling any and all requirements of the funding source, including reports, current census status updates, and other requirements established by the funding source. The employee performs a treatment function emphasizing accountability established through the funding source, and according to agency Policy. The employee is responsible for conducting risk/needs assessments and developing service plans for all clients assigned to their caseload, and implementing those plans subject to review by the Treatment Coordinator and/or approval of the Program Director. Scope of Duties: 1. Interact and communicate (verbally and nonverbally) with all clients, staff, and outside contacts professionally with respect and courtesy. 2. Maintain client and staff confidentiality in compliance with administrative policies and procedures. 3. Attend and participate in regularly scheduled staff meetings, supervisory conferences, training sessions and other meetings as assigned. 4. Orient and assess all aftercare clients via the approved agency assessment tools. 5.Complete service plans on all assigned clients based on approved agency assessment tools with client input. 6. Monitor and document client progress according to funding source standards and/or agency operational policy and procedures. 7. Update service plans with client input. 8. Schedule and facilitate case management meetings with clients as required. 9. Identify and/or develop resources and link clients to needed services. Review resource list and update quarterly as needed. 10. Facilitate scheduled workshops/programs/groups addressing client's needs. For program-specific requirements, see addendum. 11. Fulfill obligations of training agreements entered into for core curricula programs. 12. Develop appropriate discharge plans on all assigned clients as per contractual agreements. 13. Facilitate house meetings as needed. 14. Participate in required internal/external Treatment team meetings. 15. Complete all required reports and other documentation. 16. Demonstrate competency in program related certifications. 17. Provide support to treatment assistant functions. 18. Must obtain LSI-R Training and Certification within 90 days of hire. 19. Attend court hearings and/or parole hearings as subpoenaed or assigned. 20. Must be able to facilitate at least one (1) core curriculum program. 21. Conduct drug and alcohol screens as directed or needed and report results to Director. 22. Other duties as assigned. ADA Essential Functions: 1. Occasionally required to smell, stoop. 2. Regularly required to use hands to handle or feel. 3. Regularly required to reach. 4. Regularly required to stand, walk, sit, talk, and hear. 5. Regularly required to lift and move up to 10 lbs. 6. Ability to see clearly at 20 inches or less. 7. Ability to observe an area that can be seen up or down or to the left and right while remaining fixed on a given point. Credentials/Abilities: The employee in this position must have a Bachelor's Degree preferably in a Human Services related field and possess a minimum of two (2) years professional experience in mental health counseling and working with special needs populations. Employees in this position are required to have practical skills in communication, counseling processes, and resource development. Additional job-related skills include: working with addictive personalities; a basic understanding of human behavior; and utilizing group dynamics to address special needs populations. The employee must possess effective written and verbal communication skills, and an ability to act appropriately with clientele. Basic computer skills are required. The employee must demonstrate competency in core curricula facilitation and must be able to pass criminal background clearance from funding source. The employee must work flexible hours, which may include weekends and some evenings. Bilingual is a plus. Is knowledgeable of and maintains HIPPA standards of privileged information. Addendum for PROMISE: This employee is responsible for the provision of services to clients once they have fulfilled their criminal justice supervision. The employee must possess a valid driver’s license and meet agency standards. Qualifications Education Required Bachelors or better in Human Services or related field.
