RN Case Manager - In-Home Hospice & Palliative Care

Chapters Health is seeking an RN Case Manager to assess patient and family needs, coordinate the Plan of Care with the Interdisciplinary Team, and provide clinical, palliative and supportive care to keep patients at home whenever possible. The role requires an active RN license in Florida, 1+ year hospice/hospital experience, EMR/EHR proficiency, and a valid driver’s license for home visits.
