Our Nurse Practitioner will perform comprehensive health assessments which include but not limited to the following:

• Obtain history and perform physical examination
• Complete review of medicines and related instructions
• Review home safety and psychosocial needs
• Fulfill documentation needs for STAR and HCC measures
• Obtain and document all pertinent information in patient's charts and case management notes on same day as patient is seen; include medications and end of life preferences and "drivers" of preventable hospital admissions
• Instruct all patients, caregivers and family to call you prior to accessing medical care, subject to life threatening emergencies
• Maintain patient safety and confidentiality
• Universal precautions utilized in all patient care
• Prescribe/order procedures, treatments and medications within scope of practice
• Develop high level of trust and intimacy with patients and families

The skills, experience & education you bring:

• CA Licensed Nurse Practitioner
• 1+ Years of Experience with Primary Care

The base salary for this position is $135,000 - $155,000/year + sign on bonus!

The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this job description. Physical requirements: Office environment - roles involving part to full time schedule in office environment. Based in our physical offices and work from home office/deskwork – activity level: sedentary, frequency most of workday.
