Monterey Bay Vascular is a leading multi-specialty vascular and endovascular practice serving Santa Cruz and Monterey Counties. We specialize in minimally invasive arterial and venous interventions, dialysis access management, limb preservation, and advanced wound care delivered in clinic, office-based lab (OBL), and ASC settings.

Position Overview
We are seeking an experienced and motivated Nurse Practitioner (NP) or Physician Assistant (PA) to join our growing clinical team. This is a per-diem position with opportunity for expanded hours and long-term growth for the right candidate.
This role supports both vascular procedural services and longitudinal outpatient management, while also contributing to wellness and metabolic optimization programs.
Schedule:
Monday – Friday | 8:00 AM – 4:00 PM
No nights. No weekends.
Key Responsibilities
1. Comprehensive Vascular Evaluation & Management
- Perform detailed new patient consultations, including full history and physical examinations focused on arterial, venous, and dialysis access conditions.
- Evaluate patients presenting with peripheral arterial disease (PAD), chronic limb-threatening ischemia (CLTI), venous insufficiency, varicose veins, DVT history, non-healing wounds, and dialysis access dysfunction.
- Develop evidence-based treatment plans in collaboration with supervising vascular surgeons.
- Manage conservative therapy pathways, including compression therapy, wound care protocols, medication optimization, and lifestyle modification.
- Perform longitudinal management of chronic vascular patients, ensuring appropriate surveillance intervals and follow-up imaging.
- Provide clear patient education regarding disease progression, procedural options, risk factors, and prevention strategies.
2. Procedural & Peri-Operative Support
- Conduct pre-procedural evaluations, ensuring appropriate workup, medical clearance coordination, medication reconciliation, and risk stratification.
- Review imaging (duplex ultrasound, CTA, MRA, venography) and labs to confirm procedural readiness.
- Assist with day-of-procedure patient assessments and ensure informed consent documentation is complete and compliant.
- Provide post-procedure evaluations including vascular checks, access site assessment, pain management oversight, and discharge planning.
- Manage post-operative follow-up visits, including incision checks, wound care, surveillance imaging interpretation, and complication management.
- Identify early signs of complications such as hematoma, infection, thrombosis, or restenosis and escalate appropriately.
- Collaborate closely with nursing and procedural staff to maintain efficient patient flow and safety standards.
3. Dialysis Access Management
- Evaluate arteriovenous fistulas and grafts for maturation, dysfunction, stenosis, or thrombosis.
- Coordinate care with nephrologists and dialysis centers to ensure continuity and timely intervention.
- Participate in access surveillance review, including flow data interpretation and duplex results.
- Assist in pre-operative evaluation for fistula creation and post-intervention follow-up.
- Educate patients on dialysis access care and complication recognition.
4. Advanced Wound Care & Limb Preservation
- Perform wound assessments including measurement, staging, and documentation compliant with Medicare requirements.
- Develop and implement wound care plans incorporating debridement (if credentialed), advanced dressings, offloading strategies, and vascular optimization.
- Coordinate multidisciplinary care for diabetic foot ulcers, ischemic wounds, and venous stasis ulcers.
- Monitor wound healing progress and adjust treatment plans based on clinical response.
- Ensure proper documentation to support medical necessity for procedures and wound-related services.
5. Vascular Diagnostics & Clinical Data Review
- Interpret and review vascular ultrasound studies (arterial, venous, dialysis access).
- Review lab panels relevant to vascular health, including lipid profiles, inflammatory markers, renal function, and coagulation studies.
- Ensure appropriate follow-up and clinical decision-making based on diagnostic results.
- Maintain high-quality documentation aligned with Medicare and commercial payer compliance standards.
6. Wellness & Metabolic Optimization (Vascura Wellness)
- Integrate weight management, metabolic risk reduction, and nutrition counseling into vascular care plans.
- Evaluate patients for metabolic syndrome, obesity-related vascular risk, and inflammatory contributors to chronic disease.
- Support wellness consultations focused on preventative care and long-term vascular health optimization.
- Participate in structured wellness follow-up programs and patient engagement initiatives.
- Collaborate in developing individualized lifestyle modification plans that improve long-term vascular outcomes.
7. Quality Assurance & Operational Leadership
- Participate in Quality Assurance and Performance Improvement (QAPI) initiatives.
- Assist in chart audits and compliance reviews to ensure documentation accuracy and regulatory adherence.
- Contribute to clinical workflow improvements and operational efficiency initiatives.
- Support training and mentorship of medical assistants and nursing staff when appropriate.
- Maintain strict adherence to infection control protocols and patient safety standards.
8. Patient Communication & Care Coordination
- Deliver compassionate, professional bedside manner consistent with our patient-first philosophy.
- Communicate clearly with patients and families regarding diagnoses, procedures, risks, and treatment options.
- Coordinate referrals, imaging, and specialty consultations as needed.
- Ensure timely follow-up communication of results and care plans.
Qualifications
Required – Nurse Practitioner
- Current California NP License
- Furnishing Number (if applicable)
- DEA Registration (or ability to obtain)
- NPI Number
- BLS Certification
- Graduate of accredited NP program
- Meets Medicare billing requirements
Required – Physician Assistant
- Current California PA License
- Graduate of ARC-PA accredited PA program
- DEA Registration (or ability to obtain)
- NPI Number
- BLS Certification
- Meets Medicare billing requirements
Preferred
- 2–3 years experience in vascular, interventional radiology, cath lab, dialysis, or ASC/OBL setting
- Experience in procedural or surgical patient management
- ACLS Certification
- Fluoroscopy Permit (or ability to obtain within 18 months)
- Spanish bilingual skills
- Strong EMR documentation skills
Recent graduates with strong procedural exposure may be considered.
