- Graduation from an accredited medical or osteopathic school with a Doctor of Medicine (MD) or Doctor of Osteopathy (DO) degree; AND
- Completion of a residency program accredited by the American Board of Medical Specialties (ABMS) or the American Osteopathic Association (AOA).
- A current license to practice medicine in the State of Hawai'i is required. The physician must be duly licensed as a Doctor of Medicine or Osteopathy and must be approved for Active Staff membership and clinical privileges on the Medical Staff of the Hospital in accordance with the Hospital's Medical Staff bylaws and Rules and Regulations. The physician is also required to maintain Active Medical Staff membership and appropriate clinical privileges at the Hospital throughout the term of this Agreement; AND
- Membership in good standing with Kaua'i Medical Associates, an independent physician association, is required; AND
- A current, valid State of Hawai'i type #3 driver's license is preferred; AND
- Current registration with the U.S. Drug Enforcement Administration (DEA) and a current State Controlled Substance license from the State of Hawai'i Department of Public Safety's Narcotics Enforcement Division (NED) are mandatory.
- Current certifications issued by the American Heart Association (AHA) in Basic Life Support (adults, child, infant, etc.) for Healthcare Workers AND Advanced Cardiovascular Life Support (ACLS) are required (candidates for certain specialties may be required to possess current certifications issued by the AHA in Pediatric Advanced Life Support (PALS) and/or in the Neonatal Resuscitation Program (NRP)); AND
- Board certification by an appropriate certifying organization must be obtained within three (3) years of employment unless specifically exempted by the Regional Chief Executive Officer (RCEO).
