Leading a high-performing Patient Care Specialist team, the full-time remote Care Navigation Manager will oversee daily operations, ensure efficient patient case management, and act as a senior escalation point for complex situations while fostering a culture of empathy and continuous improvement. Key responsibilities Lead, coach, and develop a team to deliver exceptional patient care experiences Establish and monitor performance metrics to ensure patient satisfaction and operational efficiency Collaborate with cross-functional teams to enhance workflows and improve care quality Required qualifications 5+ years of experience in patient navigation, care coordination, or digital health operations 3-5 years of people management experience in healthcare or customer support environments Strong organizational and data-driven skills for monitoring performance metrics Demonstrated ability to handle sensitive patient information with discretion and HIPAA compliance Experience with self-funded employer benefit plans or bundled payment models is a plus
