WeCare Centers at Sugar Creek Care Center, located at 351 Causeway Drive, Franklin, PA, 16323 is actively seeking dedicated, LPNs to add to our growing team on ALL SHIFTS!!! PT/FT/PRN

FULL-TIME NEW HIRES ELIGIBLE FOR A SIGN-ON BONUS OF $5,000!

Our LPN's provide essential nursing care, including monitoring patient health, administering medications, assisting with daily activities, and collaborating with the healthcare team to ensure quality resident care.

Job Duties

• Conduct thorough assessments of each patient’s medical status upon admissions and throughout the course of treatment. 

• Provide regular patient status updates.

• Assist in implementing patient’s treatment plan under direction of the Registered Nurse.

• Complete required documentation in an accurate and timely manner.

• Supervise, administer and or assist with medications.

• Provide direct care of residents as necessary.

• Follow the plan of care for each resident.

• Ensure the residents care standards are maintained.

• Report to the Director of Nursing.

Qualifications

• Valid LPN license in the State of PA
