Work Environment

This full-time position requires the Nurse Practitioner to work onsite in a diverse setting that encompasses home care. You will typically work a 40-hour week, with core hours from 9:00 a.m. to 6:00 p.m., along with occasional evening clinics to enhance accessibility for our patients. Furthermore, participation in community outreach events and health fairs may occur on weekends, allowing you to engage with the community effectively.

Key Responsibilities

• Conduct thorough health assessments and screenings for a variety of patients, establishing a detailed health history and lifestyle background.
• Diagnose and manage both acute and chronic health conditions, emphasizing preventive care through regular patient education and health promotion.
• Collaborate with local community organizations to create and implement health education programs that target common health challenges.
• Deliver culturally sensitive care that respects the diverse backgrounds of patients, advocating for their rights and access to necessary healthcare services.

Qualifications

• A current certification as a Family Nurse Practitioner is mandatory.

Compensation and Benefits
