Work Environment

This position is based onsite, allowing you to interact directly with patients in their own environments through house calls and community facilities. You’ll work alongside a dedicated team of healthcare professionals in an innovative setting designed to foster collaboration and holistic patient care. Your standard work schedule will be Monday to Friday from 9:00 a.m. to 6:00 p.m., with some evening clinics extending to 8:00 p.m. Participation in weekend health fairs or outreach initiatives may also be required, providing further opportunities to serve the community.

Key Responsibilities

• Conduct thorough health assessments and screenings to evaluate the needs of individuals and families, ensuring customized care plans that align with their unique circumstances.
• Diagnose and manage both acute and chronic health conditions, providing essential preventive care and health promotion strategies that empower patients to take charge of their health.
• Collaborate with community organizations to develop, implement, and enhance health education programs that address pressing local health concerns and promote wellness.
• Advocate for patients' rights and access to healthcare, delivering culturally sensitive care that recognizes the unique backgrounds of each individual.
• Coordinate referrals and follow-up care seamlessly, guaranteeing continuity of care through effective communication with specialists and support services.

Qualifications

• Current Nurse Practitioner certification and an active state nursing license.
• A Master’s degree in Nursing from an accredited program.

Compensation and Benefits
