Work Environment

This role is primarily based onsite at our local hospital in Cicero, providing a supportive environment where collaboration and teamwork are at the forefront. Your standard work schedule will be from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending to 8:00 p.m. Participation in community outreach programs and health fairs may require weekend hours, promoting engagement within the community.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, gathering health histories and performing physical examinations to identify health risks.
• Diagnose and manage a range of acute and chronic medical conditions, from routine check-ups to serious health concerns, adhering to best practices in preventive care and health promotion.
• Collaborate with healthcare professionals and community organizations to develop and implement innovative health education programs tailored to the community's specific needs.
• Deliver culturally sensitive care that respects diverse backgrounds, while advocating for patients’ rights and their access to necessary health services.
• Coordinate referrals and follow-up care, ensuring continuity and quality of care for all patients.

Qualifications

• Current certification as a Nurse Practitioner, with a focus on family practice.
• Active state nursing license to practice in New York.

Compensation and Benefits

We offer a competitive salary range of $180,000 to $200,000 annually, commensurate with experience and qualifications.
