Work Environment

This position is based onsite in our outpatient office setting, which fosters a collaborative and supportive environment. You will engage daily with a diverse patient population, and your shift will typically be from 9:00 a.m. to 6:00 p.m., with the possibility of evening clinics until 8:00 p.m. Participation in community outreach events, including health fairs on weekends, may also be necessary. The average workweek consists of approximately 40 hours, allowing for a balanced work-life dynamic.

Key Responsibilities

• Conduct thorough health assessments and screenings tailored to the unique needs of individuals and families, focusing on both physical and mental health.
• Diagnose and manage a range of acute and chronic medical conditions, providing preventive care and promoting wellness.
• Collaborate with community partners to develop and implement informative health education programs that empower patients and increase health literacy.
• Deliver culturally sensitive care while advocating for patients' rights, ensuring equitable access to healthcare services.
• Coordinate referrals and follow-up care, working closely with multidisciplinary teams to maintain continuity of care for patients.
• Accurately document patient information and maintain detailed records in compliance with healthcare regulations.

Patient Population / Client Focus

This role primarily focuses on serving a diverse patient population in Cicero and the surrounding areas, emphasizing care for underserved communities. Qualifications

• Current certification as a Family Nurse Practitioner from an accredited program.
• Possession of an active and valid state nursing license.
• A Master's degree in nursing is required.

Compensation and Benefits
