Work Environment

This role is primarily onsite within a school facility located in Cicero, NY, where you will engage with students, families, and educators. The typical working hours for this position include a schedule from 9:00 a.m. to 6:00 p.m., with occasional evening clinic hours until 8:00 p.m. Participation in community outreach events, such as health fairs on weekends, may also be required as part of our commitment to community wellness.

Key Responsibilities

• Conduct comprehensive health assessments for children and their families, including physical examinations, health histories, and developmental screenings.
• Diagnose and manage a variety of acute and chronic health conditions, focusing on preventive care, vaccinations, and health promotion strategies.
• Collaborate effectively with community organizations to design and implement health education programs tailored to the unique needs of the community.
• Provide culturally competent care, advocating for patients' healthcare rights and essential services, and addressing social determinants of health.
• Ensure continuity of care by coordinating referrals, follow-up appointments, and care plans, working alongside other healthcare providers as needed.
• Maintain accurate records of patient encounters and care in compliance with HIPAA and other regulatory guidelines to uphold the highest standards of practice.
• Stay informed about public health trends and evidence-based practices to integrate into patient care and community outreach activities.

Patient Population / Client Focus

This role focuses on serving a diverse patient population, primarily students and their families within the local community. The Family Nurse Practitioner will be dedicated to addressing the unique healthcare challenges faced by children and adolescents, ensuring they receive preventive care, treatment for acute conditions, and management of chronic diseases.

Qualifications

• Current certification as a Family Nurse Practitioner from an accredited institution.
• Active state nursing license to practice in New York.
• A Master’s degree in Nursing from an accredited program.
• A minimum of 3 years of clinical experience, with a preference for backgrounds in community health or public health settings.
• Excellent interpersonal and communication skills to engage with diverse populations and build trust with patients.
• A strong commitment to social justice, health equity, and advocacy for underserved communities.

Preferred Experience

• Experience working within school health programs or community health initiatives.

Compensation and Benefits
