Job Summary:

Position Title: RN--Care Manager
Location: Apple Valley, CA 92307
Duration: 13 weeks
Work Schedule: 07:00 AM-07:00 PM-40hours
Pay rate: 68-73$/hr.

Responsibilities for this position include, but are not limited to:
• Conduct comprehensive patient assessments to identify discharge and care coordination needs.
• Develop, implement, and monitor individualized discharge plans.
• Coordinate care with physicians, nurses, therapists, social workers, and other interdisciplinary team members.
• Perform utilization review to ensure appropriate level of care and efficient resource utilization.
• Facilitate family meetings and educate patients and caregivers on the plan of care.
• Coordinate post-acute services, including home health, skilled nursing facility (SNF) placement, durable medical equipment (DME), and ambulance transportation.
• Obtain prior authorizations and coordinate with HMOs, IPAs, and insurance providers.
• Ensure compliance with CMS, Joint Commission, and other regulatory standards.
• Document patient care accurately using computerized documentation systems.
• Participate in treatment planning and support safe, timely patient transitions.
• Connect patients with appropriate community resources, including behavioral health, substance abuse treatment, homelessness services, protective services, and advance care planning when applicable.
• Collaborate with the healthcare team to improve patient outcomes and reduce readmissions.
• Maintain effective communication with patients, families, and all members of the healthcare team throughout the continuum of care.

To be considered for this position, you should have: [Skills, Education, or Experience]
• Active and unrestricted Registered Nurse (RN) license.
• Minimum 2 years of recent Acute Care Hospital Case Management experience (preferred).
• Recent experience in Patient Assessment, Care Coordination, Discharge Planning, and Utilization Review .
• Ability to independently arrange DME, Skilled Nursing Facility (SNF) placements, Home Health services, and ambulance transportation .
• Experience working with HMOs, IPAs, insurance benefits, and prior authorizations .
• Knowledge of CMS regulations, Joint Commission standards, DRGs, and discharge planning requirements .
• Experience leading or participating in interdisciplinary team meetings and family care conferences .
• Strong communication, critical thinking, and organizational skills.
• Proficiency with computerized documentation/Electronic Medical Records (EMR) .
• Experience in Emergency Department, ICU, Medical-Surgical, or Surgical Services is preferred.
• Behavioral Health-only Case Management experience does not qualify for this position.
• Current BLS Certification
