Another Chance Programs in San Tan Valley, AZ is looking for a compassionate and dedicated Direct Care Worker to join their team. Responsibilities:

• Provide direct care and daily living support to clients in a residential or community-based setting
• Assist individuals with personal hygiene, grooming, meal preparation, and daily routines
• Implement individualized care plans and document client progress accurately
• Support clients in developing life skills, independence, and community integration
• Monitor and report changes in client behavior, health, or well-being to supervisors
• Transport clients to appointments, activities, and community outings as needed
• Foster a safe, supportive, and positive environment for all individuals in care

Requirements:

• High school diploma or GED required
• Previous experience in direct care, caregiving, or human services preferred
• Valid driver's license and reliable transportation
• Ability to pass a background check and fingerprint clearance
• Strong communication, patience, and interpersonal skills
• CPR/First Aid certification (or willingness to obtain)
• Genuine compassion and commitment to person-centered care
• Ability to pass a pre-employment drug screening
