Location: New Oxford, Pennsylvania 17350

Specialty: Long-Term Care (LTC) / Nursing Home

Contract Duration: 13 Weeks

Shift: Evening/Night Shift (2:30 PM – 10:30 PM)

Shift Length: 8 Hours

Break: 30-minute unpaid break per shift

Guaranteed Hours: No guaranteed hours (3–4 shifts per week scheduled)

Key Responsibilities

• Provide direct nursing care to residents according to care plans and physician orders
• Administer medications and treatments accurately and on time
• Monitor residents' health conditions and report changes to the RN or physician
• Maintain accurate and timely documentation using Matrix charting system
• Assist with admissions, discharges, and transfers as needed
• Collaborate with interdisciplinary healthcare teams to ensure quality resident care
• Support residents with daily living activities and promote a safe environment
• Respond promptly to emergencies and resident needs

Requirements

• Active Licensed Practical Nurse (LPN) license in Pennsylvania or Multistate
• Minimum recent experience in Long-Term Care / Nursing Home setting preferred
• Strong clinical, communication, and documentation skills
• Experience with Matrix charting preferred
• Must have PA Department of Aging Fingerprint Clearance Letter on file
• Must be dependable, flexible, and able to work in a fast-paced environment
