The Family Nurse Practitioner provides comprehensive, evidence-based primary and preventive healthcare to pediatric, adolescent, adult, and geriatric patients served by the health center. The FNP performs advanced assessments; diagnoses and manages acute and chronic conditions; prescribes medications and treatments; provides health education; and coordinates care within the provider's authorized role and population focus, education, training, Texas scope of practice, prescriptive authority arrangement, and approved clinical privileges. The position emphasizes health promotion, chronic disease improvement, early intervention, culturally responsive care, and service to medically underserved populations. Essential Duties and Responsibilities Comprehensive Primary Care Position Summary The Family Nurse Practitioner provides comprehensive, evidence-based primary and preventive healthcare to pediatric, adolescent, adult, and geriatric patients served by the health center. The FNP performs advanced assessments; diagnoses and manages acute and chronic conditions; prescribes medications and treatments; provides health education; and coordinates care within the provider's authorized role and population focus, education, training, Texas scope of practice, prescriptive authority arrangement, and approved clinical privileges. The position emphasizes health promotion, chronic disease improvement, early intervention, culturally responsive care, and service to medically underserved populations. Obtain complete and problem-focused health histories; conduct comprehensive physical examinations and advanced clinical assessments; and develop differential diagnoses and evidence-based treatment plans. Evaluate, diagnose, treat, and monitor acute and chronic health conditions across the lifespan, including hypertension, diabetes, hyperlipidemia, obesity, asthma/COPD, thyroid disorders, respiratory infections, gastrointestinal and genitourinary disorders, musculoskeletal conditions, dermatologic conditions, and common behavioral health concerns. Order, perform, review, and interpret laboratory studies, EKGs, imaging, and other diagnostic tests within approved privileges. Prescribe medications, treatments, medical devices, and therapeutic interventions in accordance with Texas law, clinic policy, and the applicable prescriptive authority arrangement. Monitor clinical response, medication safety, adherence, and treatment outcomes; adjust care plans as clinically appropriate. Recognize emergencies and arrange stabilization, transfer, specialty consultation, hospitalization, or other higher-level care when needed. Provide timely follow-up for abnormal laboratory, imaging, pathology, and screening results. Care Coordination and Community-Based Care Collaborate with physicians, physician assistants, nurses, medical assistants, behavioral health providers, pharmacists, social-service staff, care coordinators, and community health workers. Coordinate specialty referrals and ensure appropriate tracking and follow-up. Assist patients in accessing medications, diagnostic services, behavioral health treatment, preventive care, social services, and community resources. Screen for health-related social needs and document appropriate interventions or referrals. Support transitions of care following hospital, emergency department, rehabilitation, or other healthcare encounters. Promote continuity of care, appropriate use of the primary-care medical home, and reduction of avoidable emergency department visits and hospital readmissions. Documentation, Quality Improvement, and Compliance Complete accurate, timely, and legally compliant documentation in the electronic health record. Document assessments, diagnoses, treatment plans, prescriptions, patient education, referrals, and follow-up requirements. Follow evidence-based clinical guidelines, standing orders, medication-management policies, and organizational procedures. Participate in population-health management, care-gap closure, patient outreach, peer review, chart review, quality-assurance activities, and patient-safety programs. Support quality measures involving diabetes and blood-pressure control, cancer screening, immunization, maternal and child health, medication adherence, and follow-up after hospitalization. Comply with HIPAA, OSHA, infection-prevention, risk-management, mandatory-reporting, and patient-safety requirements. Maintain required licensure, national certification, continuing education, prescriptive authority, DEA registration, payer enrollment, credentialing, and clinical privileges. Perform additional duties consistent with the FNP's authorized role, population focus, competency, and approved privileges. Required Qualifications Master of Science in Nursing, Doctor of Nursing Practice, or other qualifying graduate-level nurse-practitioner education. Current and unrestricted Texas Registered Nurse and Advanced Practice Registered Nurse licenses. Current national board certification as a Family Nurse Practitioner through an approved certifying organization. Authorized Family Nurse Practitioner role and population focus. Current Texas prescriptive authority, or eligibility to obtain it, and ability to maintain the required prescriptive authority arrangement with a qualified delegating physician. Current Basic Life Support (BLS) certification; ACLS or PALS may be required based on assigned duties. Successful completion of credentialing, privileging, primary-source verification, background screening, reference review, and other employment requirements. Strong clinical assessment, diagnostic, treatment-planning, patient-education, and electronic documentation skills. Ability to provide care to patients across the lifespan and communicate effectively with diverse populations. Preferred Qualifications Two or more years of experience as a Family Nurse Practitioner in primary care, family medicine, internal medicine, pediatrics, urgent care, community health, or a related outpatient setting. Experience in an FQHC, community health center, rural health clinic, public-health clinic, or medically underserved setting. Experience managing a diverse pediatric and adult patient population and a broad range of acute and chronic conditions. Experience managing high daily patient volumes while maintaining quality, safety, and timely documentation. Experience using electronic health records and population-health registries. Doctor of Nursing Practice degree and/or experience leading evidence-based quality-improvement initiatives. Bilingual or multilingual ability relevant to the community served. Physical and Scheduling Requirements Ability to perform the physical functions associated with outpatient clinical practice, including examining patients and using clinical and computer equipment. Ability to sit, stand, and walk for extended periods and respond appropriately to urgent clinical situations. Availability to work assigned clinic hours, which may include occasional evenings, weekends, community events, or travel between approved locations.
