Job DescriptionAre you a physicianwith apassion for geriatricandpost-acute care?\nVitruvian Health and Hamilton Physician Group (HPG) are seeking a Physician (MD/DO) to join our team in caring for patients and serving as the Medical Director for three skilled nursing and long-term care facilities. \nJob Highlights:\n\n Competitive Compensation\n Signing Bonus\n Up to $15k Relocation\n Student Loan Repayment Options\n Paid Malpractice Insurance\n Benefits include retirement matching,PTO, CME Allowance, and more!\n\nQualified Candidates:\n\n Board Certified/Eligible in Family Medicine or Internal Medicine;\n Preferred Board Certified/Eligible in Geriatrics\n Ideal candidate will have an interest in teaching and mentoring;\n Strong multi-tasking and time management skills;\n Ability to work as a team member; and\n Commitment to quality, patient safety and overall excellence in compassionate evidence based clinical care.\n\nOverview of Responsibilities:\n\n Provide primary care to residents in skilled nursing / long-term care settings;\n Conduct assessments, diagnostics, and manage acute & chronic conditions;\n Develop care plans aligned with individual goals & coordinate with facility teams;\n Complete Medicare re-certifications and required regulatory documentation;\n Mentor clinical staff; lead quality initiatives; implement best practices;\n Act as liaison between facility leadership and clinical teams for care coordination;\n Oversight of Advance Practice Providers in Long Term Care Facilities;\n Participation in the facilities’ Quality Assessment and Assurance committees;\n Active involvement in the process of conducting the facility assessment; and\n Responsible for implementation of resident medical care policies and coordination of physician services and medical care in the facilities.\n\n\nLocated on the campus of Vitruvian Health Care System in Dalton GA, Hamilton Long Term Care (HLTC) is comprised of 3 LTC centers with 322 beds. All three facilities provide both Short-Term rehabilitation and Long-Term Care. HLTC provides24/7 skilled nursing care; assistance with daily activities like bathing, dressing, and eating, along with three meals a day and social/recreational activities. Each facility offers wound care, physical, occupational, and speech therapies individualized forpatients recovering from surgery, illness, or injury. \nHamilton Medical Centeris a 255-bed regional acute-care hospital that offers major medical, surgical and diagnostic services, including accredited stroke and chest pain centers.
