RN Case Manager: Discharge & Care Coordination

Ochsner Health

Ochsner Health is seeking a discharge planning nurse to review pre-admission through continued stay cases and coordinate with the care team to ensure appropriate levels of care. You will communicate with ED physicians and hospitalists to support efficient patient flow and utilization management. Required RN license and BLS; Bachelor’s in Nursing preferred. 3 years of hospital-based discharge planning or case management experience is ideal; CCM/ACM certifications are a plus. #J-18808-Ljbffr Ochsner Health
