Social Work Care Manager

About the Job MedStar Health is looking for a Care Manager Social Work to join our team. As a Care Manager Social Work, you will serve as a member of the interdisciplinary care management team, providing care coordination services to Medicare FFS beneficiaries. You will coordinate, negotiate, procure, and manage patient care to facilitate cost‑effective care, patient satisfaction, and health outcomes. You will facilitate the continuum of care and work collaboratively with interdisciplinary staff inside and outside the organization. Primary Duties Advocate for members, assisting them to coordinate and access medical, psychiatric, psychosocial, and other essential services to meet their healthcare needs; provide social services to patients/clients and their families; collaborate with physicians, nurses, patients, and families to assess social needs and monitor progress of medical treatment. Maintain a working knowledge of available resources to address identified patient needs and facilitate proactive, efficient service provision. Demonstrate behavior consistent with MedStar Health mission, vision, goals, objectives, and patient care philosophy; maintain timely and accurate documentation in the case management system; participate in staff meetings, Care Management meetings, work groups, and in‑services to enhance professional knowledge. Provide input, complete assignments, and share new findings with other care team members; communicate effectively during telephonic interviews, communications with external contacts, and interactions with Case Management Specialists, Management Team, physicians, and other interdepartmental contacts. Collaborate with Primary Care Physicians, Medical Specialists, HomeHealth, and other allied providers to coordinate patient care; coordinate with community organizations and agencies to identify additional resources. Qualifications Bachelor's degree in Social Work. 1-2 years recent case management experience. 3-4 years diverse clinical experience. Valid Social Worker license in Maryland.
