**Responsibilities:**

• Provide personal care and assistance with daily living activities to residents, including bathing, dressing, grooming, and toileting
• Assist with transfers, ambulation, and mobility exercises as needed
• Monitor and report changes in resident condition to licensed nursing staff
• Maintain accurate and timely records of resident care and services provided
• Collaborate with other healthcare professionals to develop and implement comprehensive care plans
• Provide emotional support and companionship to residents, fostering a sense of community and belonging
