Ritemed is hiring a full-time Nurse Practitioner (NP) to provide on-site clinical services at Alaris Health at The Chateau in Rochelle Park, NJ. This is an urgent opening — we're looking for a Nurse Practitioner (NP) who can start soon. The Chateau is a well-established skilled nursing and rehabilitation facility in Bergen County, part of the Alaris Health network, recognized as High Performing in both short-term rehabilitation and long-term care. Prior nursing home experience is required for this position.
This is a facility-based Nurse Practitioner (NP) role — no travel between sites, no home visits. You'll be the on-site NP (Nurse Practitioner) serving a stable resident population in a supportive, well-run building. If you're a Nurse Practitioner with skilled nursing experience looking for consistency, autonomy, and a single-facility home base, this is it.
What You'll Do
Provide primary and ongoing clinical care to residents on-site at the facility
Conduct assessments, manage chronic conditions, and respond to changes in condition
Collaborate with the facility's nursing team, attending physicians, and interdisciplinary staff
Document visits and treatment plans accurately and timely
Qualifications
Active New Jersey Nurse Practitioner (NP) license
Minimum 6 months of experience as a Nurse Practitioner (NP)
Nursing home experience is REQUIRED — must have worked in a skilled nursing facility (SNF) / long-term care (LTC) setting
