In this role, you'll engage directly with individuals in need, helping them identify and meet essential needs, navigate complex systems, and find lasting housing solutions. You'll serve as the go-to resource on housing services in your area, while working with local landlords and community partners to support individuals with disabilities or those in recovery. You'll also assist clients in connecting with services related to behavioral health, physical health, and employment.

Key Responsibilities:
• Engage with individuals experiencing homelessness or housing crises to help them find safe and stable housing
• Practice Housing First, harm reduction, and low-barrier service models in daily work
• Build and maintain relationships with local landlords and educate them about community programs
• Attend Coordinated Entry/Case Conferencing meetings and collaborate with local Continuums of Care
• Assist individuals in applying for benefits and accessing behavioral health, substance use, and medical services
• Provide support during transitions to permanent housing and help clients prepare for independence
• Use SOAR training to assist individuals in benefit applications
• Re-engage individuals who have disconnected from services or are at risk of returning to homelessness
• Support case managers, clients, and internal teams with up-to-date housing resources
• Maintain accurate and timely data entry per Department of Mental Health requirements
• Help clients obtain identification documents needed for housing or employment
• Understand and apply HUD Quality Standards inspections

Education, Experience, and/or Credential Qualifications:
• Bachelor's degree in Social Work, Psychology, Sociology, or a related field
OR
• Associate of Applied Science in Behavioral Health Support from an approved institution
OR
• Qualified to provide Community Support, including:

• A mental health professional as defined in 9 CSR 10-7.140(2)(QQ)
• An individual with a bachelor's degree in human services (Social Work, Psychology, Nursing, Education, Criminal Justice, etc.)
• Any four-year degree and two years of qualifying experience
• Any four-year combination of higher education and qualifying experience
• Four years of qualifying experience

Additional Qualifications:

• Must have a valid driver's license and use of personal vehicle for transportation
• Must maintain current auto liability insurance, including uninsured motorist coverage
• Must complete SOAR program and Housing Liaison training from the Department of Mental Health
• Completion of HUD Quality Standards quiz required
• Must be able to handle confidential information and work independently
• Strong Microsoft Office skills and the ability to multitask in a fast-paced environment

Physical Requirements:
• ADA Consideration - Sedentary work: Exerting up to 10 pounds of force occasionally (up to 1/3 of the time) and/or a negligible amount of force frequently (1/3 to 2/3 of the time) to lift, carry, push, or pull, or otherwise move objects, including the human body. Repetitive movements of hands, fingers, and arms for typing and/or writing during work shift.

• Sedentary work involves sitting most of the time but may involve walking or standing for brief periods of time. Jobs are sedentary if walking and standing are required only occasionally, and all other sedentary criteria are met.
