Home Health Registered Nurse (RN) - Medicare / OASIS

Responsibilities Conduct patient evaluations in homes and rehab facilities Complete OASIS assessments (SOC, ROC, Discharge) Develop, implement, and coordinate patient care plans Determine appropriate visit frequency and needed services Maintain accurate, Medicare-compliant documentation Collaborate with intake, billing, clinicians, and caregivers to coordinate services Travel throughout the assigned territory to support patient care Credentials Active RN license in good standing within Massachusetts Required Qualifications Prior OASIS experience or be able to complete OASIS training (see below) 1+ year of home health or community nursing experience Familiarity with Medicare regulations Strong clinical assessment and critical-thinking skills Reliable transportation and willingness to travel throughout the territory OASIS Training Opportunity Don’t have OASIS experience yet?
