Case Manager

Full Time, Professional – Wilmington, MA, US Salary Range: $26.00 to $27.50 annually Case Manager We offer our full‑time employees Medical, Dental and Vision coverage, 100% paid by the company for employees and family members. Previous experience with substance use disorder treatment and case management is required. The Case Managers at Mayflower Recovery are responsible for providing confidential addiction counseling. The team provides education and support to individuals, families, and the community. They complete all required clinical documentation for their caseload, promote healthy lifestyles, and under the supervision of the Clinical Supervisor ensure post‑treatment placement to the most appropriate level of care. Responsibilities include: Complete all clinical documentation for caseload including Biopsychosocial assessment, treatment plans, individual progress notes and collateral contact with supports and aftercare sources. Develop the patient’s discharge/transfer plans to ensure movement to the appropriate levels of care. Contact referral sources, provide case consultation and coordination with referral sources. Coordinate ongoing care for housing and therapeutic needs with the Treatment Team, substance abuse treatment providers, healthcare providers, community resources, and family members. Participate as a member of a multidisciplinary team for assigned patients. Document all counseling and education services, assessments, reassessments, referrals and follow‑up in the patient’s clinical record, providing appropriate signatures and dating of such entries, including those made in electronic records. Case Managers may also perform the work of an Addiction Counselor including: Obtain previous records that are relevant to the current treatment episode. Collaborate with patient to develop a written treatment plan that is patient‑centered and recovery‑oriented and includes goals and measurable objectives. Provide the substance abuse counseling services specified in the patient’s treatment plan. Review the patient throughout the treatment episode according to ASAM, to determine the need for continued services or discharge/transfer. Review and, where necessary, revise the substance abuse counseling portion of the patient treatment plan to address emerging problems. Complete Biopsychosocial assessments with assigned patients. Provide active case consultation. Assess, plan services, and deliver services to individuals in need of substance abuse treatment. Establish rapport, systematically gather data, determine readiness for individual treatment and change, and apply accepted criteria for diagnosis of substance use disorders. Participate in continuing professional development. Know medication‑assisted therapy. Maintain state‑required staff‑to‑patient ratio. Provide education and information to individuals and groups through formal and/or informal processes concerning their substance abuse. Identify patient needs that cannot be met by the counselor or agency and assist patients to utilize the support systems and community resources available.
