This role focuses on helping individuals improve communication, cognitivefunction, and swallowing abilities-right where they feel most comfortable. Your Impact in This Role: Communication & Cognitive Therapy: Evaluate and treat speech, language, and cognitive-communication disorders. Support patients with memory, attention, and problem-solving challenges. Swallowing & Safety Management: Assess and treat swallowing disorders (dysphagia). Provide recommendations to improve safe eating and reduce aspiration risk. Personalized Care Delivery: Develop therapy plans tailored to each patient's home environment and goals. Modify interventions based on progress and clinical findings. Patient & Caregiver Education: Teach strategies that patients and caregivers can apply between visits. Promote long-term carryover of communication and swallowing techniques. Documentation & Coordination: Complete visit notes using a digital EMR system. Communicate with care team members as needed to support patient outcomes.
