Hospice Social Worker - Per Diem

Key Responsibilities

• Provide psychosocial support, counseling, and education to hospice patients and their loved ones.
• Conduct assessments to identify emotional, social, and financial needs.
• Collaborate with nurses, aides, chaplains, and physicians as part of an interdisciplinary care team.
• Assist families with advance care planning, community resources, and bereavement support.
• Advocate for patient rights, dignity, and preferences.
• Maintain accurate, timely documentation in accordance with agency and regulatory standards.

Qualifications

• Licensed Social Worker (LSW/LCSW/LMSW or equivalent state licensure).
• Experience in hospice or healthcare setting preferred.
• Exceptional communication, counseling, and advocacy skills.
• Compassion, flexibility, and the ability to support patients and families through emotional transitions.
• Valid driver’s license and reliable transportation required.
