Physical Therapist - Home Health

Schedule
- Shift: 8-hour days only
- Weekends: Not required

Assignment Details
- Guaranteed Hours: Facility may call off up to one shift per pay period

Requirements
- License: Pending license accepted
- Certifications: BCLS required
- Experience: 1 year of skilled home health experience required
- Unit-specific skills: Home health physical therapy skills including orthopedic post-operative care, neuro rehabilitation, gait and mobility training, home exercise programs, patient and family education, safe body mechanics

Roles & Responsibilities
- Provide patient assessments and develop appropriate treatment plans in accordance with industry standards.
- Implement and supervise therapeutic interventions that aim to improve patients' physical functioning.
- Educate patients and families regarding treatments and techniques for home exercises and physical rehabilitation.
- Document patient progress and outcomes in compliance with facility standards.
- Collaborate with interdisciplinary teams to optimize patient care and coordinate rehabilitation strategies.

Compliance / Notes
- Charting systems: Experience with OASIS required; familiarity with electronic charting systems is expected.
- Orientation/modules: Pre-start modules required.
- Transportation: Clinician expected to provide own reliable transportation.
- Vaccine: Confirm COVID-19 vaccine requirement with hiring manager before submission.
