Position Responsibilities:

• Evaluate and treat residents in a skilled nursing facility setting
• Develop individualized treatment plans
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members
• Complete timely and compliant documentation
• Help patients achieve their highest level of functional independence
