Geriatrics Nurse Practitioner

Nurse Practitioner / Advanced Practice Provider - Mishawaka, IN

Compensation: $120,000 - $140,000+ base salary (range flexible based on experience; higher end available for language-critical areas) Performance bonus up to $8,000 Sign-on bonus flexible

Schedule: Monday - Friday, 8 am - 5 pm

This position sits within a Medicare-focused value-based care model built around outcomes not volume. You'll join a fully supported care team, see a sustainable panel, and be rewarded for keeping patients healthy and out of the hospital. Bonuses are structured to reward low hospital admissions, preventive care, and strong patient experience.

Practice Details

• Patient volume: 13 - 16 per day
• Visit length: 20 minutes standard / 40 minutes for new patients and full diagnostics
• Visit frequency: approximately every 3 months (complex cases seen more frequently)
• Care team: Scribe, Medical Assistant, Social Worker
• On-site resources: In-house lab with phlebotomist, RubiconMD specialist access
• EMR: Canopy and Greenway
• Providers per clinic: 3 - 5 (mix of NPs and PCPs)
• Patient transportation: Van pickup available within 5 miles of clinic
• Community room with patient programming: exercise classes, yoga, spa days, and social events

Qualifications

• MSN, DNP, or NP Certificate Program graduate
• National certification in one of the following: Family Nurse Practitioner (FNP), Adult-Gerontology Primary Care NP, Adult NP, or Gerontological NP
• Active, non-probationary state NP license
• Active DEA license
• HCC documentation, ICD-10 coding, and HRA experience preferred
