Job Details

We are seeking a Physician to provide primary care services to a senior population in a collaborative, patient-centered care environment. This role focuses on chronic disease management, preventative care, care coordination, and helping older adults maintain their health and independence. Physicians work closely with an interdisciplinary team to deliver comprehensive care and improve patient outcomes. Responsibilities:

• Provide primary medical care, including comprehensive assessments, treatment planning, and ongoing care management.
• Diagnose and manage acute and chronic medical conditions in an elderly and medically complex population.
• Collaborate with nurses, specialists, and interdisciplinary team members to coordinate patient care.
• Order and interpret diagnostic tests and adjust treatment plans as appropriate.
• Provide education and support to patients and families regarding disease management and preventative care.
• Supervise and collaborate with nurse practitioners and physician assistants.
• Maintain accurate and timely clinical documentation.

Qualifications:

• Board Certified or Board Eligible in Family Medicine or Internal Medicine.
• Minimum of 1 year of experience working with an elderly or frail population.
• Active California Physician License.
• DEA registration required.
• Experience in geriatrics, primary care, skilled nursing, long-term care, home-based care, managed care, or senior-focused healthcare preferred.
• BLS certification required.
• Strong communication and interdisciplinary teamwork skills.
