Physical Therapist (PT) – Full-Time
West River Civita Care Center
Milford, CT
$45–$50/hour

West River Civita Care Center is seeking a motivated Physical Therapist (PT) to join our rehabilitation team. This full-time opportunity is ideal for a clinician who enjoys helping residents regain mobility, improve strength, and return to greater independence in a supportive skilled nursing and post-acute care environment.

What You'll Do

• Perform physical therapy evaluations and develop individualized treatment plans
• Provide skilled therapy focused on mobility, strength, balance, gait, transfers, and endurance
• Monitor resident progress and adjust treatment plans based on clinical needs
• Educate residents and caregivers on exercises, mobility techniques, and safety strategies
• Collaborate with PTAs, OTs, COTAs, SLPs, nursing, and other members of the interdisciplinary team
• Complete accurate and timely clinical documentation

Qualifications

• Active Connecticut Physical Therapist license required
• Graduate of an accredited Physical Therapy program
• Strong clinical, communication, and teamwork skills
• Skilled nursing or rehabilitation experience preferred but not required
• New graduates are encouraged to apply
