Lockport Rehab & Health Care Center is hiring a Speech-Language Pathologist (SLP) to join our therapy team at 909 Lincoln Avenue. We're an 82-bed skilled nursing and rehabilitation center — big enough to give a Speech-Language Pathologist a real, varied caseload, small enough that you'll know your residents by name and they'll know yours. This is a hands-on Speech-Language Pathologist role covering short-term rehabilitation, long-term care, and our outpatient program. Prior nursing home experience is preferred — if you've worked in a skilled nursing facility (SNF) or long-term care (LTC) setting, you'll hit the ground running here. That said, we're glad to talk with strong Speech-Language Pathologists coming from hospitals, home care, or outpatient practice who want to move into skilled nursing. You'll work alongside Physical Therapy (PT), Occupational Therapy (OT), nursing, and dietary as part of a genuine multidisciplinary team, with modern therapy equipment and leadership that listens to its clinicians.

What You'll Do

• Evaluate and treat residents for speech, language, cognition, voice, and swallowing (dysphagia) disorders

• Build individualized plans of care and adjust them as residents progress

• Manage a caseload across short-term rehabilitation, long-term care, and outpatient services

• Complete dysphagia evaluations and make diet texture and consistency recommendations in coordination with nursing and dietary

• Document evaluations, daily treatment, and progress accurately and on time, in line with Medicare and Medicaid requirements

• Contribute to care plan meetings and Minimum Data Set (MDS) input as it relates to speech and swallowing

• Train residents, families, and staff on communication strategies, swallowing precautions, and safe feeding techniques

• Coordinate daily with the interdisciplinary team (IDT) — PT, OT, nursing, dietary, and social services

Qualifications

• Master's degree in Speech-Language Pathology from an accredited program

• Current New York State license as a Speech-Language Pathologist (SLP), or license-eligible in New York State

• Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA), or in the process of completing it

• Nursing home experience preferred — prior work in a skilled nursing facility (SNF) or long-term care (LTC) setting is a real plus

• Dysphagia evaluation and treatment experience

• Comfortable documenting in an electronic medical record (EMR)

• Solid communication skills with residents, families, and the interdisciplinary team (IDT)
