SNF PT Opportunity – Winter Haven, FL

Ardor Health Solutions is seeking a Physical Therapist for a 13-week Skilled Nursing Facility travel assignment in Winter Haven, Florida .

This is a full-time opportunity working with an adult and geriatric rehabilitation population. You'll help patients regain strength, mobility, balance, and independence following hospitalization, surgery, illness, or injury.

Qualifications

• Graduate of an accredited Physical Therapy program
• Current Florida PT license or ability to obtain prior to start
• Current BLS certification
• Strong evaluation and treatment skills
• Comfortable treating adult and geriatric patients
• SNF or geriatric rehab experience preferred
• Strong documentation and communication skills

What You'll Do

• Complete PT evaluations and reassessments
• Develop individualized plans of care
• Provide gait, balance, strengthening, transfer, and mobility training
• Address endurance, safety, and functional limitations
• Provide therapeutic exercise and functional training
• Educate patients and caregivers
• Provide fall-prevention and safety education
• Collaborate with PTA, OT, COTA, SLP, nursing, and other members of the care team
• Assist with discharge planning
• Complete timely clinical documentation
