This role serves as the connection between hospitals, physicians, case managers, discharge planners, referral sources, patients, families, and our facilities to ensure smooth transitions of care while building strong community partnerships. The ideal candidate has experience in healthcare marketing, admissions, community outreach, long-term care, or hospital liaison services and enjoys developing relationships that support quality patient care and facility growth.
