RN Care Coordinator

$34.14 - $61.23 per hour

RN Case Managers may be eligible for a relocation bonus up to $8,000. The RN Care Coordinator is responsible for overseeing the progression of care and discharge planning for identified patients requiring these services. The RN Care Coordinator performs this role to meet the individual's health needs while promoting quality of care, cost‑effective outcomes and by following hospital policies, standards of practice and Federal and State regulations. The position’s emphasis will be on care coordination, communication and collaboration with utilization management, nursing, physicians, ancillary departments, insurers and post‑acute service providers to progress the care toward optimal outcomes at the appropriate level of care. The RN Care Coordinator advocates for the patient and family by identifying, valuing, and addressing patient choice, spiritual needs, cultural, language and socioeconomic barriers to care transitions. In addition, the RN Care Coordinator strives to enhance the patient experience. Job Requirements Graduate of an Accredited School of Nursing. Nursing Diploma or Associates Degree in Nursing required, BSN preferred. RN License in applicable state of employment required. Minimum two (2) years of acute hospital clinical experience or a Master’s degree in Case Management or Nursing field in lieu of acute experience. At least five years of total nursing experience required. Certified Case Manager (CCM), Accredited Case Manager (ACM‑RN), or UM Certification preferred. Experience working with EMR preferred. Working knowledge of regulatory requirements and accreditation standards strongly preferred. BLS from the American Heart Association required within three months of hire if located within hospital. Physical Requirements – Medium Work – (Exert up to 50 lbs force occasionally, and/or up to 20 lbs frequently, and/or up to 10 lbs constantly). Where You’ll Work Located in Lakewood, Colorado, St. Anthony Hospital is a Level I Trauma Center that provides a full range of medical specialties and health‑care services to Denver and the surrounding region through our state‑of‑the‑art medical campus and home base for Flight For Life Colorado. With four trauma rooms including the T‑10 room, a dedicated field‑to‑surgery suite ready 24/7 for trauma surgeons and specially trained teams, our staff can provide life‑saving care to the most severely ill and injured patients. In 2023, we received our Magnet designation and many other awards, certifications and accreditations from the American Heart Association, Chest Pain Center, American College of Cardiology, The Joint Commission, the Emergency Nurses Association, NAPBC, American College of Radiology and many others.
