Join our team at Harmony Hospice as a Home Health Aide.  

Full-time opportunity available

$23.50-$26.00 per hour

At Harmony Hospice, we believe compassionate care brings comfort, dignity, and peace to patients and their families. The Home Health Aide provides personal care and supportive services to hospice patients under the supervision of a Registered Nurse, helping ensure comfort, safety, and quality of life throughout their care journey.

Responsibilities for Home Health Aide (Hospice):

• Provide personal care and activities of daily living, including bathing, grooming, dressing, toileting, oral hygiene, and mobility assistance.

• Assist patients with positioning, transfers, ambulation, and other comfort measures as directed by the Registered Nurse.

• Observe and report changes in the patient's physical, emotional, or environmental condition to the supervising nurse.

• Accurately document care provided and patient observations in accordance with hospice policies and regulatory requirements.

• Promote patient comfort, safety, dignity, and independence while following the individualized plan of care.

• Provide compassionate emotional support to patients and their families throughout the hospice journey.

• Maintain a clean, safe, and organized patient environment.

• Collaborate with the interdisciplinary hospice team to ensure continuity of care and exceptional patient outcomes.

• Perform other duties as assigned.

Qualifications for Home Health Aide (Hospice):

• Must hold a current Home Health Aide license in Rhode Island .

• Must meet all state Home Health Aide training requirements, including completion of at least 75 hours of classroom and supervised practical training.

• Current BLS/CPR certification required.

• Previous hospice, home health, or patient care experience preferred.

• Understanding of the hospice philosophy and ability to provide compassionate end-of-life care.

• Ability to work independently with minimal supervision.

• Compassionate, dependable, and emotionally mature with empathy for individuals who are ill, frail, or terminally ill.

• Valid driver's license with reliable, insured transportation required.

• Excellent communication, observation, and documentation skills.
