About the Role:

The Supervisor of Care Coordinators and Case Management plays a pivotal role in overseeing and guiding a team dedicated to delivering comprehensive care coordination and case management services within the healthcare and social assistance sector. This position ensures that care coordinators effectively manage patient cases, facilitate communication among healthcare providers, and connect clients with necessary resources to optimize health outcomes. The supervisor is responsible for maintaining high standards of service delivery, compliance with regulatory requirements, and continuous quality improvement initiatives. By fostering a collaborative and supportive team environment, the supervisor enhances staff performance and professional development. Ultimately, this role contributes to improved patient satisfaction, efficient care transitions, and the overall success of the care management program across the organization.

Minimum Qualifications:

• Bachelor’s degree in Nursing, Social Work, Healthcare Administration, or a related field.
• Minimum of 3 years of experience in care coordination or case management within a healthcare or social services setting.
• At least 1 year of supervisory or team leadership experience.
• Strong knowledge of healthcare systems, case management principles, and relevant regulatory requirements.
• Proficiency in electronic health records (EHR) and case management software.

Responsibilities:

• Lead, mentor, and supervise a team of care coordinators and case managers to ensure effective case management and care coordination.
• Develop and implement policies, procedures, and best practices to enhance service delivery and compliance with healthcare regulations.
• Monitor caseloads, review case plans, and ensure timely and accurate documentation and reporting.
• Collaborate with healthcare providers, social service agencies, and community resources to facilitate comprehensive care for clients.
• Conduct performance evaluations, provide ongoing training, and support professional development opportunities for team members.
• Analyze program data and outcomes to identify areas for improvement and implement quality improvement initiatives.
• Address and resolve complex client issues and escalations in a timely and effective manner.
• Participate in interdisciplinary team meetings and contribute to strategic planning and program development.
