Occupational Therapist (OT) - Home Health | Per Diem

Job Description

@Home Therapy is looking for a compassionate and motivated New York State licensed Occupational Therapist to provide one-on-one care to adult and geriatric patients in their homes.

This opportunity is ideal for an OT who enjoys independence, flexibility, and making a meaningful difference in patients' daily lives.

Responsibilities Include:

Completing in-home OT evaluations and treatment.
Developing individualized treatment plans.
Addressing daily living skills, mobility, coordination, cognition, and home safety.
Educating patients and caregivers on adaptive techniques and equipment.
Monitoring progress and updating care plans.
Completing EMR documentation and collaborating with the care team.
