Occupational Therapist (OT) – Home Health Location: Indianapolis, IN Schedule: Flexible / Per Diem / Full-Time Options Available

About the Role: We are currently seeking a compassionate and motivated Occupational Therapist (OT) to join our home health team. In this role, you'll have the opportunity to make a meaningful impact in the lives of patients by helping them regain independence and improve their quality of life-all in the comfort of their own homes.

As a Home Health OT, you'll be responsible for evaluating patients, developing individualized treatment plans, and implementing interventions tailored to support each patient's goals, whether it's increasing mobility, mastering daily living skills, or ensuring a safe home environment. You'll work collaboratively with a multidisciplinary team including nurses, physical therapists, and speech-language pathologists to ensure comprehensive care.

Key Responsibilities:

• Conduct initial and ongoing assessments to determine patients' functional status, needs, and goals
• Develop and implement customized therapy plans to support rehabilitation and independence
• Provide training and education to patients, families, and caregivers on adaptive techniques and equipment
• Monitor progress and adjust care plans as needed to achieve desired outcomes
• Ensure accurate and timely documentation in compliance with regulatory standards
• Communicate effectively with the care team to coordinate treatment and improve outcomes
• Recommend and assist with the acquisition of adaptive equipment and home modifications to improve safety
• Participate in discharge planning and provide guidance on continued care or community resources

Qualifications:

• Licensed Occupational Therapist in the state of Indiana (required)
• Graduate of an accredited OT program
• Prior experience in home health preferred but not required
• Strong communication, critical thinking, and time management skills
• Ability to work independently in a home-based setting
